to 2. i've gotten em on a 650 word fic before (the title is a meme so it was pretty funny to see "i loved well well well... the conses of my quences, it was so deep and moving") and 3. have gotten a couple of them days after posting.
the only constant is that they try to get you off site, afaict.
Recently got one on a 564 word fic a few days after posting but this one was craftier and acted like they wanted to give me writing tips, likely jumping me with the commission part after luring me into a conversation about something else.
