This likely should be developed more, but it's a fill. Sherlock can smell pregnancy, and this leads to a deduction at a crime scene, sniffing of surrogates, shouting at Anderson, and reveals Sherlock's fascination with Roman intrigue. I hope it checks all of the OPs boxes.
