Sherlock with an especially 'King of the Assholes' kind of attitude at a crime scene. John keeping quiet until they're out of sight, then he shoves Sherlock up against the wall of an alley, or into a shuttered alcove, and telling him in quiet, explicit tones exactly how Sherlock will make it up to him. More intimate than any flagrant public indecency. Then John just walks away.
-which hit my inbox exactly 5 minutes before I left for home from my field post, right before an hour and a half car ride in the middle of a really fucking boring part of the state, and all I could think of on the way home was filling it.

John finds out their relationship isn't quite what he thinks it is. But it isn't quite what Sherlock thinks it is, either.

Betadine is cold, always. It’s a law of the universe. Sherlock flinches lightly as the technician swirls circles of the disinfectant over his chest. He’s nervous – years of intravenous drug use hadn’t exactly inured him to the pain of a needle sliding its way into his skin, but the payoff at the end was worth it, the bliss of complete oblivion. From the intent and heated look on John’s face, he’s pretty sure the reward for this bit of painful endurance will be just as good, if not better.

This little thing does not need to be read in order with any other part of the series (Come Into My Parlor, Dark of the Alley, Come Undone, and Whatever You Ask For), but knowledge of their relationship would inform it a little bit.
“I want to play a little game,” he says, and the fluttery anticipation blooms to white-hot arousal in an instant. John knows how to counter Sherlock's boredom - a little bit of risk, a little mental challenge, and a lot of self-discipline.
