Medicine cat is a hereditary position (and the rank's been renamed to augur). Augurs arrange marriages and transfer their kids across clan lines as a way to form alliances. The senior augur in each clan goes by the title of matriarch or patriarch and is in charge of managing the bloodline. Kits of augurs form part of a caste known as patrician and have special privileges because of their lineage going back all the way to Moth Flight and her kits.

Summary
After Moth Flight's kits are sent to different Clans, it becomes tradition to send apprentice seers to a new Clan, where they finish their training and live for the rest of their lives.
The tradition is said to prevent unnecessary bloodshed and violence, as well as remind seers that their allegiance is to the unity of the Clans, not any individual Clan.
That doesn't make it easy.

They’re somewhere in Croatia, and Sam’s been hard at work, trying to pull Steve away from a busy night of sketching and staring at his phone. He wants to go out and drink plum brandy and dance with the locals.

It’s small – three floors, and three very poky flats to a floor, except for the bottom floor that was the entrance hall and her own quarters – but that just meant she knew everybody who lived here, and liked them.
