After John Carter quits surgery to restart his residency in the ER, Peter Benton is furious. He refuses to hear Carter's apologies, his excuses. Empty words.
But then Carter is brutally attacked on his first day as an ER resident. Strangled by a patient. His injuries are terrible. The boy cannot breathe on his own. He needs surgery to repair his vocal cords.
With Carter unable to speak, Peter must make the extra effort to repair what has broken between them.

“Stay back!” Peter points at the man and bellows.
He rushes to Carter’s side, and after haphazardly kicking away some glass shards, kneels down on one knee.
The stench of Scotch assaults his nostrils immediately. The sleeves and shoulders of Carter’s suit jacket are soaked in it.
Peter reaches out to rest a hand on Carter’s shoulder. The man on the floor flinches back so hard that Peter loses his balance and has to catch himself with one hand.

The beat of the chest compressions bangs in his head like a sledgehammer. Hands clenched together, arms aching, he pumps against Carter’s heart. Seconds tick by. How long has it been? He feels something give way, under his hands. On the next compression, a distinctive crack sounds through the room. Peter doesn’t let up. Broken ribs are nothing, now. An afterthought. He keeps going, staring fiercely at the heart monitor. Straight line. He keeps going. Carter’s chest inflates up and down with artificial breaths. He pumps against it. Again and again and again.
“Don’t be an asshole, Carter,” Peter snarls. Or pants. He can feel heavy beads of sweat gathering on his forehead. He pays them no mind. “Don’t you dare die.”
OR: A freak accident during a trauma sends Carter into sudden cardiac arrest.

Neil wakes with a familiar feeling settling in his chest, rattling in his bones. He can smell the cigarette smoke, the burning flesh, the bloody fingers and all of the above mash together, helpful to create a hazy impression of Neil's surroundings.
He isn’t sure if he woke up hurting, or if the wave of phantom pain came with the choking fear. He only knows to push himself up from the bed, eyes wide and focused on the door, like he knows someone is behind it.
Neil sometimes gets a tingle, an anxious little thing that keeps him alert and alive when his mother is not near. She should be watching the door now.
She should be, but when Neil glances to his side he has to brace himself to not move at all. Andrew is asleep beside him.

Jack Abbot: veteran, emergency medicine Senior Attending for night shift; and Michael Robinavitch: Chief Emergency Medicine Attending of the day shift, have been inseparable for years. Viewed as best friends to the rest of what's grown to be another family in the Pitt.
They've been in a relationship for a huge chunk of the two decades of knowing each other-living together for almost a decade. No one knew because they didn't feel the need to make a big thing about their relationship. They loved quietly to most, saving the tenderness for behind closed doors, which they are genuinely alright with.

She tosses the plastic bottle into the buggy just as Tanner says, “...and Mel says that ponies aren’t baby horses, they’re something different. A baby horse is called a colt.”
“That’s right,” Abby says automatically, the words snagging on long-buried memories of Saddle Club and Misty of Chincoteague. And then, a delayed second later: “Who’s Mel?”
Because she usually does listen when he talks, and she thought she knew the names of all of his friends and all of his friends’ siblings and that she and Frank have both trained him well enough that he wouldn’t call an adult by their first name without some kind of title in front of it. But she definitely doesn’t remember hearing about a Mel before. A new kid in class? Or, God, a character from one of the more annoying shows he and Penny watch, the kind whose shrillness and obnoxious flashing make Abby flee the room?
She absolutely isn’t expecting the explanation Tanner gives.
“Daddy’s friend.”
The buggy jerks to a stop, the broken wheel—there’s always a broken wheel—dragging across the linoleum. Penny giggles at the sound it makes, but Abby doesn’t hear her, her mind suddenly gone blank.
